Referral Process

How Do I Submit A Referral?
Referrals for the Inclusion Coach Program must be submitted by licensed child care programs and/or EarlyON Programs. If your program is seeking guidance on inclusive practices or to support a child’s unique needs, our Inclusion Coaches are here to provide hands-on support, professional development, and tailored strategies to help educators meet the diverse needs of the children in their care.
​For Child Care Programs who do not have a child who requires a referral to the Inclusion Coach Program, they can access capacity building for educators on strategies, coaching, professional learning and inclusion practices by contacting Family Space to complete a Request For Support.
If you are a parent of a child who may benefit from the support of an Inclusion Coach, we encourage you to discuss this with your child’s child care program or EarlyON Educator. For EarlyON Referrals, you and your child must attend EarlyON Programs on a regular basis.

PROFESSIONAL DEVELOPMENT
Provide professional development and building on existing capacity of child care/EarlyON programs; Professional Learning can be tailored to support a child’s unique inclusion needs.



ENVIRONMENT/PEDAGOGY SUPPORT
ENHANCED
SUPPORT
TRANSITION TO SCHOOL
Adaptation suggestions to child care/EarlyON program's environment/pedagogy to include all children within the program
Can be requested if deemed required and if funding is available.
Transition to school support is done in collaboration with families, child care, Inclusion Coach, and school boards.

DEVELOPMENTAL ASSESSMENTS
Completion of ASQ’s by Inclusion Coaches, who are trained facilitators. Referrals to appropriate community agencies in collaboration with child care, EO programs, and families.

DEVELOPMENTAL INCLUSION PLANS
If the child has complex inclusion needs, a D.I.P. will be created to provide developmental strategies to be implemented in child care and at home.
